What Health System Pharmacy contributes to Cardinal Health

Health System Pharmacy is responsible for providing customized pharmacy program solutions that reduce costs and improve patient care quality for hospitals, health systems and other integrated healthcare providers may also consult with and advise healthcare team on prescribed medications, supplies and related processes.

The clinical MTM Pharmacist will provide patient-centered MTM services for the purpose of optimizing medication therapy to improve clinical, economic and humanistic outcomes. This will be accomplished by providing the services described below that may be delivered telephonically, by remote video link and/or at times in-person with the patient:

Accountabilities:

• Comprehensive Medication Reviews

• Targeted Interventions

• Patient-Centered Adherence/Compliance Services

Qualifications

  • Bachelors degree in pharmacy required, PharmD preferred
  • 19 hours or less per week on average
  • 2-4 years related pharmacy experience
  • MTM experience preferred
  • Ability to become licensed pharmacist in state(s) of pharmacy operation

Additional Desired Qualifications:

  • Completion of Delivery of MTM Services Training Certificate program
  • Board Certification in Ambulatory Care or Pharmacotherapy
  • Pharmacist licensure in multiple states

  • Demonstration of professional conduct at all times while collaborating with healthcare providers, health plans, patients, and/or caregivers
  • Ability to utilize approved MTM platforms for documentation
  • Applies working knowledge in the application of concepts, principles, and technical capabilities to perform varied tasks
  • Works on projects of moderate scope and complexity
  • Identifies possbile solutions to a variety of technical problems and takes actions to resolve
  • Applies judgment within defined parameters
  • Receives general guidance may receive more detailed instruction on new projects
  • Work reviewed for sound reasoning and accuracy
